Geriatric Care Management (GCM)
Are you faced with caring for an elderly loved one and feeling overwhelmed by it all? Are you considering a nursing home or assisted living facility but are not sure which is best? If that’s the case, Geriatric Care Management is another aspect of home health care that you may want to consider instead.
Geriatric Care Managers serve as “project managers”, so-to-speak, for individual patients. Their job is to see the long-term care process from beginning to end, keeping the family in the loop all along the way. They make assessments of the individual’s needs and make recommendations on care that may be most appropriate. They also refer the individual to knowledgeable professionals who will assist them in getting their affairs in order including: elder law attorneys, trust officers, medical professionals, and financial advisors.
QHS is a member of the National Association of Geriatric Care Managers. We are partners with Geriatric care managers throughout the state of Florida to be sure that our clients every needs are met. QHS partners with Geriatric Care Managers (GCM) who will determine the person’s needs, plan and coordinate care and services, and monitor the situation to ensure the plan is on track. And if living at home is no longer feasible, a GCM can assess other options.
GCM services includes:
- Oversee every aspect of doctors' visits and hospitalization
- Care assessment and planning to identify problems and provide solutions
- Screen, arrange and monitor in-home help or other services
- Short- or long-term assistance for caregivers living near or far away
- Review financial, legal or medical issues and offer referrals to geriatric specialists
- Provide crisis intervention
- Oversee care and communicate with families at a distance, quickly alerting them to problems
- Assist with moving an older person to an appropriate living environment
- Provide consumer education and advocacy
- Offer counseling and support
- Medication management
- Is the Liaison between all Healthcare and other providers to ensure clients wellness and well being.
