QHS undergoes a rigorous selection process for the recruitment of caregivers. Our SMART Hire™ process Screens for Measurable Attributes to Retain the Top caregivers. You should know that once you have joined our team, you have joined because you are among the best and will be respected and treated as such by management and staff.
In order to join the QHS team, you need the following original documents and requirements:
- Application form
- Professional Licenses/Certificates
- Two Professional Reference Letters
- Current CPR Card within the last two (2) years
- Driver’s License/State ID
- Social Security Card
- Employment Authorization (e.g. Alien Card, Voter’s Registration Card, Passport or Work Permit)
- Original 4 Hours of HIV Certificate
- 4-12 Hours of In-Services (HIV/AIDS, OSHA, Assisting with Medication, Domestic Violence (CEUs, LPN & RN Only)
- Current Physical/PPD or chest X-ray (within the last 6 months, which states you are Free of Communicable Diseases)
- Car Insurance (if driving required)
- Drug Testing
- Nationwide Background Check
- Signed consent form permitting us to conduct state and county criminal Background Checks (if you haven’t already done so at another agency within 2 years).
